Do we take your plan?
Search the plan name printed on your card. One insurer can run several networks, and being in one does not mean being in another — so “Cigna” or “MVP” on its own is not enough to go on. Every plan below is listed with the products we do not participate with.

Check before you come
Still not sure? Don’t guess.
Employers change administrators, networks get renamed, and the card in your wallet does not always say which network you are in. Call with the card in hand and we will check your exact plan before you come in — no appointment needed to ask.
What the list means
Before you book
Finding your plan above answers one question. These four answer the rest.
- 01
Read the plan name, not the logo
One insurer can run several networks. Being in one does not mean being in another, so we check the plan name printed on your card — not the brand on the front.
- 02
Exclusions are part of the answer
Where a plan above lists exclusions, those named products are out of network even though the insurer itself is listed. Match your card against them before you book.
- 03
In network is not the same as covered
In network means we hold a contract with your plan. What you owe — deductible, copay, coinsurance, referrals, prior authorization — is set by your plan, not by us.
- 04
Participation can change mid-year
Contracts are renegotiated and plans move networks during the year. This page reflects our current participation; confirm your benefits with your insurer before your visit.
Bring these to your visit
- 01
Photo ID
A driver's licence or any government-issued photo identification.
- 02
Every current card
Primary and secondary coverage both. If your plan changed since your last visit, tell the front desk before you are seen.
- 03
A referral, if your plan needs one
Some plans require one before a specialty visit. Check with your plan to see if you need a referral before you come in.
This page is a participation list, not a quote. Being in network sets what we are paid, not what you owe. Confirm your benefits with your plan before your visit. No insurance? Ask about self-pay pricing at Care Today.
