Vaccine supply and eligibility follow current CDC and New York State Department of Health guidance. Call us if you have questions about whether a dose or booster is right for you.
Complete your NYS form
Complete the required New York State vaccination form and bring it with you to your appointment.

Before You Arrive
How to Prepare for Your Vaccine
A little preparation helps your visit go quickly and smoothly.
- Complete your NYS Vaccine Form and bring it with you.
- Bring and wear a mask during your vaccination.
- Bring your identification, proof of residency, and proof of eligibility.
- Get a good night's rest, drink plenty of fluids, and eat before your appointment.
- Bring your smartphone, if you have one.
- If possible, bring a friend or loved one with you.
- Prepare for a 15-minute observation period following the vaccine.
- Wear comfortable clothing that makes your arm easily accessible, such as a tee shirt.
